• **Document**: You will accurately document client care and any issues that may arise, ensuring that our clients' files are up-to-date and complete.
• **Collaborate**: You will work closely with our Care Coordinators, Nurses, and other healthcare professionals to ensure that our clients receive comprehensive and continuous care.
