Care Manager – Flexible Caseload & Competitive Pay:
We are seeking a dedicated HH Care managers to provide individualized support and coordination for clients. This role offers a meaningful opportunity to make a difference while maintaining a flexible caseload.
Responsibilities:
Develop and implement care plans tailored to client needs
Coordinate services and resources to ensure optimal support
Maintain accurate and timely documentation
Communicate effectively with clients, families, and providers
Qualifications:
Experience in care management, social work, or a related field preferred
Strong organizational and communication skills
Ability to manage multiple cases efficiently
Must have a BA
