Summary:
Peerstar is seeking an experienced Care Coordinator to join our Operations Team. This role is ideal for candidates with a background in home care coordination, scheduling, staffing, client services, case management support, or healthcare operations.
The Care Coordinator serves as the link between service delivery, staffing, and peer engagement. This position is responsible for coordinating services, matching peers with Certified Peer Specialists, managing schedules, monitoring authorized service hours, and ensuring continuity of care.
Candidates with experience in home health care, home care agencies, personal care services, caregiver scheduling, Medicaid programs, behavioral health, or healthcare staffing are strongly encouraged to apply.

Responsibilities:
Staff peers with Certified Peer Specialists based on IRP authorization, availability, and geographic needs
Monitor utilization to ensure authorized hours are maximized
Identify and resolve service gaps or underutilization trends
Conduct outreach to support peer engagement and retention
Collaborate with CPS and leadership to maintain consistent service delivery
Track scheduling trends and recommend operational improvements
Maintain compliant documentation and follow HIPAA standards
Qualifications
1-3 years of experience in Care Coordination, Home Care Scheduling, Healthcare Staffing, Client Services, Case Management Support, or Healthcare Operations.
Previous experience working in a Home Care Agency, Home Health Agency, Personal Care Agency, Behavioral Health Program, or Medicaid-funded program preferred.
Experience coordinating schedules, managing service hours, or staffing field-based employees strongly preferred.
Strong organizational, communication, and problem-solving skills.
Ability to prioritize multiple tasks in a fast-paced environment.
Proficient with Microsoft Office and electronic documentation systems.
Experience working with Medicaid, managed care, authorizations, or healthcare scheduling is a plus
