Pay Rate: $20.00 per hour
Schedule: Part-Time (30 Hours per Week)
Rose View Nursing and Rehabilitation Center is seeking a caring, organized, and resident-focused Social Services Assistant to support the emotional, psychosocial, and discharge planning needs of our residents. This position plays an important role in enhancing resident quality of life while assisting the Social Services Department in providing comprehensive support services.
We are particularly interested in candidates who have previous experience working as an Activity Aide in a skilled nursing facility, rehabilitation center, assisted living community, or long-term care setting, as this experience provides valuable insight into resident engagement and quality of life programming.
ResponsibilitiesAssist residents and families with adjustment to the facility and available resources.
Support the Social Services Director with assessments, care planning, and documentation.
Help coordinate discharge planning and transitions of care.
Assist with resident advocacy and addressing psychosocial concerns.
Maintain accurate and timely documentation in accordance with facility policies and regulatory requirements.
Participate in interdisciplinary care team meetings.
Help facilitate resident activities, family communication, and community resource referrals.
Assist residents with applications, benefits, and access to community services as appropriate.
Promote resident rights, dignity, and quality of life.
Associate degree in Social Work, Human Services, Psychology, Sociology, or a related field preferred.
Previous experience in healthcare, skilled nursing, long-term care, social services, or human services preferred.
Prior experience as an Activity Aide is strongly preferred.
Strong communication, interpersonal, and organizational skills.
Ability to work effectively with residents, families, healthcare professionals, and community agencies.
Knowledge of resident rights and long-term care regulations is a plus.
