Salary: $70K-75K/Annually

Job Summary:

We are seeking a compassionate and experienced Social Services Director to join our Skilled Nursing Facility in Palm Beach County, Florida. As a key member of our leadership team, you will play a vital role in ensuring the highest quality of life for our residents, and their families. We are committed to maintaining a facility where compassionate healthcare, nursing, and rehabilitation services are provided in an environment of respect, dignity, and personal attention to the needs of the individual.

Responsibilities:

• Develop and implement effective social services programs and services that meet the needs of our diverse resident population

• Collaborate with the interdisciplinary team to ensure seamless care coordination and delivery

• Provide emotional support and counseling to residents and family members

• Develop and maintain relationships with community resources, organizations, and referral sources

• Recruit, train, and supervise social services staff and volunteers

• Participate in quality improvement initiatives to ensure continuous improvement and excellence in social services

• Maintain accurate and timely records and reports
