Key Responsibilities
  • Organize and maintain records: File and retrieve patient records in both paper and electronic formats (Sigma)

  • Process admissions/discharges: Handle paperwork including consent forms, insurance documents, and discharge summaries

  • Data entry and updates: Input demographic, medical history, and treatment data into patient charts

  • Record accuracy: Review and correct records for completeness and compliance with regulations

  • Confidentiality: Protect patient information in accordance with HIPAA and employer policies

  • Chart management: Prepare charts for appointments, monitor chart flow, and maintain secure tracking when records are out of the medical records room

  • Communication: Respond to patient inquiries about records and coordinate with healthcare staff for information needs

  • Quality control: Participate in chart review committees and implement record-keeping quality measures

Qualifications
  • Education: High school diploma or equivalent; associate degree or certification in health information technology, medical terminology, or related fields is preferred

  • Experience: Prior experience in a medical office or healthcare setting; familiarity with Sigma software

  • Skills:

    • Proficiency in computer systems and office software

    • Strong attention to detail and organizational skills

    • Knowledge of HIPAA regulations and release of information policies

    • Excellent verbal and written communication

    • Ability to work independently and in a team environment
