We are a 50-bed skilled nursing facility in the heart of Greenfield, Ohio, dedicated to serving our residents and families with excellence. The Social Services Designee/Activity Director is a key part of our leadership team—ensuring smooth transitions into care, managing relationships with referral partners, and helping us shine in the community.

Responsibilities for Social Services Director/Activity Director

·       Develop and maintain relationships with residents, family members, and caregivers to understand their needs and preferences

·        Identify and address any issues or concerns related to our activity program or residents' care

·        Participate in interdisciplinary team meetings to share information and collaborate on resident care

·        Stay up-to-date with industry trends and best practices in activity programming and social work

·       Complete monthly activity calendars

·       Help assist resident and family with a safe discharge

·        Represent Greenfield Skilled Nursing and Rehabilitation at community events and conferences

Requirements for Social Services Designee/Activity Director

·       Develop and maintain relationships with residents, family members, and caregivers to understand their needs and preferences

·       Identify and address any issues or concerns related to our activity program or residents' care

·        Participate in interdisciplinary team meetings to share information and collaborate on resident care

·        Stay up-to-date with industry trends and best practices in activity programming and social work

·        Represent Greenfield Skilled Nursing and Rehabilitation at community events and conferences

Job Types for Admissions Director

·        Full Time

·        Day Shift

·        Occasional evenings or weekends for facility events

·        Manager on Duty (some weekends)
