About the Role:

As a Social Services Director, you will be responsible for overseeing the social services department and ensuring that our residents receive the highest quality care. Your expertise will enable you to develop and implement individualized care plans that cater to the unique needs and preferences of each resident. You will work closely with our interdisciplinary team to ensure that our residents receive comprehensive care that addresses their physical, emotional, and social needs.

Responsibilities:

• Oversee the social services department, ensuring that all services are delivered in accordance with state and federal regulations
• Develop and implement individualized care plans that address the unique needs and preferences of each resident
• Conduct assessments and evaluations to identify resident needs and develop strategies to meet those needs
• Recruit, train, and supervise social services staff, including social workers, activity programmers, and other support staff
• Collaborate with other departments, including nursing, rehabilitation, and dietary services, to ensure seamless delivery of care
• Participate in care plan conferences and other interdisciplinary team meetings to ensure coordination of care
• Maintain accurate and up-to-date records and reports, including resident assessments, care plans, and service reports
• Ensure compliance with all state and federal regulations, including those related to social services and care plans

Requirements:

• Bachelor's degree in Social Work, Psychology, or a related field
• Minimum 1 years of experience in social services, preferably in a long-term care setting
• Strong knowledge of social services regulations, guidelines, and best practices
• Excellent communication, interpersonal, and leadership skills
• Ability to work effectively in a fast-paced environment with diverse populations
