Responsibilities:
- Provide personalized support and assistance to individuals with disabilities in daily living activities.
- Develop and implement individualized care plans to promote independence and well-being.
- Facilitate social interactions and community engagement opportunities.
- Monitor client progress and report any changes in behavior or health to the supervisor.
- Assist with personal care tasks, including hygiene, meal preparation, and medication management.
- Maintain accurate documentation of services provided and client progress.
- Collaborate with families and other professionals to ensure comprehensive care.
- Participate in ongoing training and professional development opportunities.
Requirements:
- High school diploma or equivalent; relevant certifications preferred.
- Experience in direct support or caregiving roles is highly desirable.
- Strong communication skills and a compassionate demeanor.
- Ability to work independently and as part of a team.
- Valid driver's license and reliable transportation.
- Background check and drug screening required.
- Flexibility to work various shifts, including evenings and weekends.
