- Provide skilled physical therapy services to patients in their homes
- Develop individualized treatment plans to improve mobility and independence
- Collaborate with an interdisciplinary healthcare team
- Educate patients and caregivers to support long-term success
- Deliver compassionate, patient-centered care throughout the community
- Active Physical Therapist (PT) license
- Current BLS certification (if required)
- Valid driver's license and reliable transportation
- Ability to work independently while managing a daily patient schedule
- Home Health experience
- Strong communication and documentation skills
- Experience working independently in a community-based setting

for the following shift(s): Days
