The Occupational Therapist-Home Health in Northern AZ plays a key role in delivering personalized therapy services to primarily adult patients within their homes. As part of an interdisciplinary team, this position involves frequent travel to multiple residences, focusing on patient assessment, care planning, and implementing effective therapeutic strategies to enhance patients’ daily living skills. The therapist also educates patients and caregivers, monitors progress, and ensures safety through appropriate recommendations and documentation.

Responsibilities

• Conduct patient assessments to identify therapy needs
• Develop individualized care plans tailored to patient goals
• Implement therapeutic techniques during home visits
• Educate patients and caregivers about therapy and safety
• Monitor and document patient progress regularly
• Collaborate with interdisciplinary team members for coordinated care

Preferred Qualifications

• Minimum 1 year experience in occupational therapy
• Licensed Occupational Therapist (OT)
