Nurse Practitioner – Primary Care
Location: Bronx, NY
Job Type: Full-Time | Permanent
Salary: $135,000 – $150,000 per year (Based on Experience)
Position Overview
We are seeking a compassionate and dedicated Primary Care Nurse Practitioner to join a growing multidisciplinary healthcare team in the Bronx, NY. This role offers the opportunity to provide comprehensive primary care services to patients of all ages while collaborating with physicians and other healthcare professionals to deliver exceptional, patient-centered care.
Key Responsibilities
- Perform comprehensive physical examinations and patient assessments.
- Diagnose, treat, and manage acute and chronic medical conditions.
- Develop individualized treatment plans based on patient needs and clinical findings.
- Order, interpret, and evaluate laboratory tests, diagnostic imaging, and other medical studies.
- Prescribe medications and treatments in accordance with New York State regulations.
- Provide preventive healthcare services, including routine screenings, immunizations, and health education.
- Maintain accurate and timely documentation in the electronic medical record (EMR/EHR).
- Collaborate with physicians, specialists, and multidisciplinary teams to coordinate patient care.
- Educate patients and families on disease prevention, medication management, nutrition, and healthy lifestyle practices.
- Refer patients to specialists and community resources when appropriate.
- Ensure compliance with clinical guidelines, quality standards, and regulatory requirements.
Qualifications
- Master's Degree in Family Nurse Practitioner (FNP) or Adult-Gerontology Nurse Practitioner (AGNP) program.
- Active New York State Nurse Practitioner License.
- National Board Certification (AANP, ANCC, or ONCC).
- Strong clinical assessment and diagnostic skills.
- Experience with EMR/EHR systems preferred.
- Bilingual (Spanish) is a plus but not required.
