Astyra is seeking Medicaid Assessment Nurses (RNs) to conduct in-person assessments for individuals with Intellectual and Developmental Disabilities (IDD) and Autism throughout Clarion, Indiana, and Armstrong Counties, Pennsylvania.

What You'll Do

\n

\n
• Conduct in-person Medicaid assessments using the SIS-A assessment tool
\n
• Meet with individuals, caregivers, and support teams in community settings
\n
• Evaluate functional needs and document assessment findings accurately
\n
• Complete and submit reports electronically within required timeframes
\n
• Travel throughout Clarion, Indiana, and Armstrong Counties as needed
\n
• Maintain professional communication while providing compassionate, person-centered service
\n

Qualifications

\n

\n
• Active Registered Nurse (RN) license
\n
• Experience working with individuals with Intellectual and Developmental Disabilities (IDD), Autism, behavioral health, long-term services and supports, or case management preferred
\n
• Strong assessment, documentation, and communication skills
\n
• Ability to work independently and manage your own schedule
\n
• Reliable transportation and willingness to travel within the assigned counties
\n
• Comfortable using technology to complete electronic documentation
\n
