Qualifications & Experience Requirements for Licensed Social Worker:

Previous Experience as a Social Worker preferred
Social Worker License

Qualifications for Licensed Social Worker:

Psychosocial assessments/BIMS/PHQ9/Trauma Induced Care
Attend Care plans/meetings
MOLST review
Must have Knowledge of HCPOA/Advanced Directives, PASSAR/ID screens
Work with the Interdisciplinary team with regards to discharge planning, MDS completion, formulate individualized care plans
Provide resources PRN
Advocate for our Seniors Aging in Place
Attend meetings as appropriate
