Job Description
The Clinical Liaison is responsible for driving facility growth through strategic marketing, referral development, and community engagement. This role builds and maintains strong relationships with hospitals, physicians, discharge planners, and post‑acute providers to ensure seamless patient transitions and increased admissions. The Clinical Liaison also supports the admissions process by gathering required clinical and referral information and conducting pre‑admission assessments when appropriate.
Key ResponsibilitiesReferral Development & Relationship ManagementBuild and sustain strong relationships with acute‑care hospitals, healthcare systems, discharge planners, case managers, rehabilitation centers, assisted living facilities, hospices, and home care agencies
Maintain ongoing communication with physicians, social workers, and other referral sources
Collaborate closely with hospital discharge planners and physicians to ensure smooth patient transitions
Maintain responsibility for patient referrals through completion of the admission process
Demonstrate a strong understanding of inpatient rehabilitation and LTACH admission criteria
Conduct on‑site pre‑admission assessments as needed
Utilize fast‑track referral processes to support timely conversions
Partner with regional marketing teams and facility leadership to develop and implement effective marketing strategies
Create and support marketing materials, including brochures, flyers, advertisements, and campaigns
Participate in community events and outreach activities to support organizational growth and brand awareness
Provide input and leadership in strategic planning initiatives
Identify opportunities to enhance services, improve referral relationships, and increase census
LPN or RN license preferred
Minimum of two (2) years of experience in the licensed field, ideally within rehabilitation, LTACH, or a related post‑acute setting
Prior marketing or business development experience, preferably in senior living, skilled nursing, or healthcare
Strong interpersonal, communication, and relationship‑building skills
Ability to engage physicians, healthcare professionals, and community partners professionally and tactfully
Approximately 80% local travel within the assigned market
