We are seeking a dedicated Occupational Therapist OT to help us deliver outstanding rehabilitation services to our residents.  

Your Role as an Occupational Therapist OT: 

At Poplar Hill Health and Rehab, our Occupational Therapist OT provides essential care to our residents by: 

  • Completes evaluation of residents for rehab services in accordance with building policies. 

  • Administers treatment to patients in compliance with physician’s prescriptions or referral after reviewing patient’s history and progress report. 

  • Performs procedures such as range of motion, manual muscle test, ADL, sensory motor, sensation, perception and adapts tasks as needed. 

  • Writes and charts progress notes as required and reports patient’s status in weekly scheduled meetings. 

  • Reports any significant change in patient’s status to appropriate personnel, e.g. nursing or physician. 

  • Instructs the patient or family members, nursing staff and others with respect to treatment procedures or programs, safety precautions, home exercise program, etc., and submits written instructions to patient and family members when indicated. 

  • Reports needed supplies and repairs of equipment to the Director of Rehab and assists in housekeeping duties and caring for equipment. 

  • Performs all tasks and duties in an efficient and safe manner. 

  • Performs other related duties as assigned or as necessary. Remains flexible and adaptable in work schedules and work assignments as defined by departmental and facility needs. 

What You Need: 

  • Must have graduated from an accredited Occupational Therapy program
