Purpose and Impact:

This position’s primary role is to provide direct clinical care and coordination of health services for youth in the program, including supporting medical, dental, and behavioral health needs. The Medical Coordinator provides information related to treatment and care, ensures timely documentation, and works under the guidance of the Lead Medical Coordinator in collaboration with medical providers, Program Director, Assistant Program Director, and other related departments.

Essential Functions:

  • Assist in the delivery of medical services to youth, ensuring services are provided in compliance with organizational policies and procedures, ORR requirements, and applicable State and Federal guidelines.
  • Prepare youth for examinations and complete medical intakes, including recording medical history, vital signs, height, and weight; assist with appropriate triage for routine lab testing (e.g., pregnancy testing) in accordance with established timelines.
  • Assist medical providers during examination procedures, including patient screening and triage as needed.
  • Administer medications under provider orders in accordance with established medication administration, storage, and documentation protocols.
  • Update youth medical records, including allergies, medication use, and immunization history, ensuring accuracy, completeness, and confidentiality in accordance with HIPAA and ORR requirements.
  • Administer immunizations under standing orders from licensed providers and in accordance with established guidelines.
  • Educate and counsel youth regarding health conditions, treatment plans, medication use, and disease prevention, as directed by providers.
  • Respond to youth health-related questions in accordance with provider guidance and established procedures.
  • Identify and report health concerns, including urgent or emergent conditions, to the Lead Medical Coordinator or medical provider and assist in coordinating appropriate response.
  • Assist in scheduling and coordinating medical, dental, and behavioral health appointments, ensuring compliance with required timelines and follow-up.
  • Support continuity of care by assisting with referrals and coordination of services with external providers and community partners.
  • Maintain accurate and complete medical records for each youth, including required documentation and maintenance of hard copy and electronic files as applicable.
  • Maintain and review inventory of medical supplies, prescribed medications, and over-the-counter medications; notify appropriate staff when supplies need to be replenished.
  • Assist in preparing and maintaining required reports, including weekly, monthly, quarterly, and annual documentation, and support audit and monitoring activities.
  • Support implementation of medication management processes, including medication counts, labeling accuracy, and self-administration protocols, including over-the-counter medications.
  • Coordinate and assist with transportation of youth to medical appointments as needed.
  • Complete and submit documentation related to Significant Incident Reports involving medical concerns in accordance with established procedures.
  • Participate in Quality Improvement activities and follow established protocols to support compliance and service delivery.
  • Bilingual (English/Spanish) is required 

Other Functions:

  • Able to react to change productively and handle other essential tasks as assigned.
  • Develop and maintain effective communication and working relationships with staff, co-workers, physicians, and youth.
  • Develop and expand medical, specialty and dental services with local providers.
  • Maintain Health Information Privacy HIPAA.

Physical Requirements and Environmental Conditions:

  • Ability to remain in a stationary position (sitting or standing) for extended periods while completing documentation, medication administration, and monitoring activities.
  • Ability to move about the facility frequently, including walking between resident rooms, common areas, and outdoor spaces.
  • Ability to position self to provide care, including bending, stooping, kneeling, crouching, and reaching.
  • Ability to lift, move, and/or assist residents with mobility needs, including occasional lifting of up to 30–50 pounds (with or without assistance or adaptive equipment).
  • Ability to use hands and fingers for clinical tasks such as administering medications, operating medical equipment, and using a computer or documentation system.
  • Ability to communicate effectively (verbally and in writing) with youth, families, and interdisciplinary team members.
  • Ability to observe and assess residents, including recognizing behavioral, emotional, and physical health changes.
  • Ability to respond quickly to emergencies, including physical intervention when necessary in accordance with training and policy.
  • Work is performed in a residential group home setting with youth who may have behavioral, emotional, or medical needs.
  • Regular exposure to indoor residential environments and occasional outdoor activities.
  • Potential exposure to communicable diseases, bodily fluids, and medical waste, requiring adherence to infection control protocols.
  • Exposure to unpredictable or crisis situations, including verbally or physically escalated behaviors.
  • Work may involve non-traditional hours, including evenings, weekends, holidays, and overnight shifts.
  • Moderate noise levels with occasional elevated noise or activity levels during youth interactions or crisis situations.
  • Use of standard healthcare and office equipment, including electronic health records systems.

Education:

  • Register Nurse degree from an accredited college or university in Nurse or Health Sciences; RN License according to State requirements.
    • Must have graduated from an approved RN nursing program approved by the National Council of State Boards of Nursing (NCSBN)
    • Must have passed the appropriate NCLEX-RN Exam

Experience:

  • Minimum of one (1) year of experience in a healthcare, public health, or clinical setting required.
  • Demonstrated experience in medical assessment, basic triage, and treatment skills with diverse populations of children and adolescents required.
  • Experience in a hospital, clinic, medical records department, or physician’s office preferred.
  • Experience working with youth, including those with behavioral health needs, trauma histories, or in residential or youth services settings preferred; may include relevant part-time, volunteer, or internship experience.
  • Experience supporting populations with complex medical, behavioral health, or trauma-related needs preferred.

Knowledge, Skills & Abilities:

  • Ability to perform clinical assessments and basic triage, and to respond appropriately to routine and urgent health concerns within scope of practice.
  • Knowledge of pediatric and adolescent health practices, including common medical and behavioral health conditions.
  • Ability to follow established clinical protocols, medication administration procedures, and infection control standards.
  • Strong organizational skills with the ability to manage multiple tasks and meet required timelines.
  • Knowledge of medical documentation standards, including accuracy, completeness, and confidentiality requirements (HIPAA).
  • Effective communication and interpersonal skills, with the ability to interact with youth, staff, and medical providers.
  • Ability to identify changes in health status and escalate concerns to the Lead Medical Coordinator or medical provider as appropriate.
  • Ability to work effectively in a fast-paced, high-demand environment while maintaining attention to detail.
  • Proficiency in electronic medical records and other healthcare-related systems.
  • Ability to maintain control and work under pressure to meet deadlines.
  • Ability to work with diverse populations, including youth with varying cultural, linguistic, and clinical needs.
