We are seeking an experienced and compassionate Case Manager Registered Nurse (RN) to join the Purchased Referred Care (PRC) department at the Fort Peck Service Unit. This contract opportunity is ideal for an RN with strong case management, utilization review, and care coordination experience who is dedicated to ensuring high-quality patient care and efficient healthcare resource management within the Indian Health Service (IHS) system.
\nThe Case Manager RN will collaborate with providers, patients, specialists, and multidisciplinary healthcare teams to coordinate referrals, monitor patient care plans, and facilitate access to medically necessary services.

- Coordinate and manage patient care through the Purchased Referred Care (PRC) program. \n
- Review referrals to determine medical necessity and eligibility in accordance with PRC policies and guidelines. \n
- Develop, implement, and monitor individualized patient care plans. \n
- Collaborate with physicians, specialists, hospitals, and community healthcare providers to ensure continuity of care. \n
- Perform utilization review and case management activities to promote quality outcomes and cost-effective care. \n
- Educate patients and families regarding treatment plans, available healthcare resources, and follow-up care. \n
- Maintain accurate, timely, and compliant clinical documentation within the electronic health record (EHR). \n
- Monitor patient progress and coordinate transitions of care. \n
- Participate in interdisciplinary meetings and quality improvement initiatives. \n
- Ensure compliance with all federal, state, IHS, HIPAA, and organizational policies. \n

- Active, unrestricted Registered Nurse (RN) license in any U.S. state or territory. \n
- Current BLS (AHA) certification. \n
- Minimum 2 years of recent RN Case Management experience. \n
- Experience in utilization review, discharge planning, or care coordination preferred. \n
- Strong knowledge of clinical documentation standards and healthcare regulations. \n
- Excellent communication, organizational, and critical thinking skills. \n
- Ability to manage multiple priorities in a fast-paced healthcare environment. \n
- U.S. Citizenship required. \n

- Previous experience within the Indian Health Service (IHS), Tribal Health, or Federal Healthcare facilities. \n
- Experience with Purchased Referred Care (PRC) programs. \n
- Familiarity with electronic medical record systems. \n
- Certification in Case Management (CCM or ACM) is a plus. \n
