\n • Chief Redstone Health Center – Wolf Point, MT

CONTRACT TERM:
\n • 12-Month Assignment
\n • 2,080 Hours (1.0 FTE)
\n
SCHEDULE:
\n • Monday – Friday
\n • 8:00 AM – 4:30 PM (Local Time)
\n • Excluding Federal Holidays
\n • Schedule coordinated with the PRC Supervisor
\n
POSITION SUMMARY:
\n The Nurse Case Manager (PRC) is responsible for providing comprehensive case management and care coordination services for patients with special healthcare needs. The RN collaborates with providers, Purchased/Referred Care (PRC) staff, and external healthcare facilities to ensure medically necessary, cost-effective, and appropriate healthcare services while promoting continuity of care and positive patient outcomes.
\n
ESSENTIAL RESPONSIBILITIES:
\n • Provide specialized case management services and coordinate care for patients with complex and special healthcare needs.
\n • Review and monitor outpatient consultations and referrals for medical necessity, appropriateness, and cost-effectiveness.
\n • Coordinate referrals and follow-up services with healthcare providers, specialists, hospitals, and external facilities.
\n • Collaborate with PRC staff and interdisciplinary teams to ensure care aligns with CMS guidelines and organizational requirements.
\n • Monitor and track patient referrals, authorizations, and deferred consults to ensure timely access to care.
\n • Maintain accurate statistical data, reports, and documentation related to consults, referrals, and case management activities.
\n • Facilitate continuity of care and advocate for patient needs throughout the referral and treatment process.
\n • Participate in quality improvement initiatives and support the continuous improvement of PRC processes and outcomes.
\n • Maintain accurate and timely documentation in accordance with organizational, state, and federal regulations.
\n • Ensure compliance with HIPAA, confidentiality standards, and all applicable policies and procedures.
\n • Communicate effectively with patients, families, providers, and external healthcare organizations regarding treatment plans and referral status.
\n • Participate in departmental meetings, training programs, and professional development activities.
\n • Perform additional duties and responsibilities as assigned.
\n
MINIMUM QUALIFICATIONS:
\n • Graduate of an accredited Registered Nursing program.
\n • Current, active, and unrestricted Registered Nurse (RN) license.
\n • Minimum of three (3) years of recent Registered Nurse experience preferred.
\n • Previous Case Management and/or Purchased/Referred Care (PRC) experience preferred.
\n • Experience in utilization review, care coordination, discharge planning, or community health nursing preferred.
\n
KNOWLEDGE, SKILLS, AND ABILITIES:
\n • Strong knowledge of case management principles and care coordination practices.
\n • Knowledge of CMS guidelines, medical necessity criteria, and utilization review processes.
\n • Excellent assessment, critical-thinking, and problem-solving skills.
\n • Strong organizational and time-management abilities.
\n • Excellent communication and interpersonal skills.
\n • Ability to work independently and collaboratively within a multidisciplinary healthcare team.
\n • Proficiency with electronic health records and healthcare documentation systems.
\n • Strong patient advocacy and customer service skills.
\n
ADDITIONAL REQUIREMENTS:
\n • Must comply with all federal contract requirements, including HIPAA, PPE, background checks, computer security, documentation standards, and professional appearance requirements.
\n • Overtime must be pre-approved by the COR or Service Unit TPOC and appropriately documented.
\n • Must maintain accurate logs of hours worked and support continuous service coverage as required by the facility.
