Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.
A rehab hospital pharmacy is seeking a dedicated Staff Pharmacist to support patient care in an inpatient setting. This position offers a Monday through Friday schedule, with primary coverage from 12:30 PM to 9:00 PM, and shared weekend, after-hours, and on-call responsibilities with the pharmacy manager. The role is contract to hire, with potential for full-time employment if the fit is right. Pharmacy hours are expanding to 8:00 AM through 9:00 PM, reflecting the growing needs of the facility.
The facility operates 30 staffed beds with an average daily census of 28 patients. The pharmacy team currently includes two pharmacists and one technician, utilizing Wellsky as the hospital pharmacy system and Pyxis for automation.
Key Responsibilities:
- Review and verify medication orders to ensure accuracy and appropriateness
- Prepare, dispense, and verify medications, including limited sterile IV compounding (no chemotherapy)
- Monitor and adjust patient medication therapy, incorporating lab values and clinical data
- Collaborate effectively with physicians, nurses, and other healthcare staff
- Ensure compliance with hospital policies, and state and federal pharmacy regulations
- Provide drug information and counsel patients and healthcare professionals
- Supervise and support pharmacy technicians and other team members
- Manage controlled substances and maintain pharmacy inventory
- Lead medication safety initiatives and document medication-related incidents
- Participate in quality assurance and ongoing pharmacy operations
- Stay current on pharmacy best practices and clinical guidelines
Qualifications:
- Active and clear Wisconsin pharmacist license
- Experience with IV compounding (non-chemo) required
- Previous exposure to hospital inpatient pharmacy preferred
- Ability to work collaboratively in a small pharmacy team environment
