Job Summary:

Watsontown Nursing and Rehab center, "Welcome home to Watsontown", is seeking a dedicated and compassionate LPN - Licensed Practical Nurse to join our team. As a valued member of our nursing staff, you will play a crucial role in delivering high-quality patient care and supporting our mission to provide exceptional rehabilitation and long-term care services.

About the Role:

• Provide direct patient care, including administering medications, performing treatments, and assisting with daily living activities

• Collaborate with interdisciplinary teams to develop and implement individualized care plans

• Conduct patient assessments and document as required

• Participate in quality improvement initiatives to enhance patient outcomes

• Demonstrate a strong commitment to patient-centered care and customer service

Responsibilities:

• Administer medications, treatments, and therapies as prescribed by physicians

• Assist patients with activities of daily living, such as bathing, dressing, and eating

• Monitor patients' conditions and report any changes or concerns to the RN or physician

• Participate in patient education and discharge planning

• Maintain accurate and up-to-date patient records
