Position Summary: Provides rehabilitative services to children/youth under the age of 21 who are Medicaid eligible that meet medical necessity. All Children and Family Treatment and Support Services (CFTSS) services can be delivered in the community where the child/youth lives, attends school, and/or engages in services. CPST services are goal-directed supports and solution-focused interventions intended to achieve identified goals or objectives per the child/youth, family/caregiver, or other collateral supports. CPTS is a comprehensive service and includes but is not limited to: Intensive Interventions (counseling), crisis avoidance (counseling), Immediate Team crisis management (counseling) provided by a Master's degree CPST provider. CPST services also may include the delivery of Rehabilitative Psychoeducation, strength-based service planning, and rehabilitative supports; these services may be provided by a Bachelor's or master's level CPST provider.

Primary Duties & Essential Functions:

  • Comply with all requirements of CFTSS
  • Comply with all requirements of 291 Li censure
  • Compliance with applicable laws, rules, and regulations, to include CFTSS guidelines.
  • Participate in the intake and planning process and assessment as requested.
  • Master level CPST: Provide Intensive Interventions (counseling), crisis avoidance (counseling), Intermediate Term crisis management (counseling).
  • Provide intensive Interventions (counseling) - individual, family, and relationship-based counseling. Engage the child/youth and family/caregiver in ways that support the everyday application of treatment methods as described in the treatment plan.
  • Provide crisis avoidance (counseling) - assist the child/youth with effectively responding to or avoiding identified precursors or triggers, identifying potential crisis, developing a crisis management plan and/or seeking other supports to restore stability and functioning. Assist the child and family in developing the capacity to prevent a crisis episode or reduce the severity of a crisis episode should one occur.
  • Provide Intermediate Term crisis management (counseling) - assist families following a crisis episode. Assist to stabilize child/youth in the home and natural environment, assist with the goal setting on issues identified from emergency room intervention and other referral sources.
  • Provide rehabilitative psychoeducation - educating the child/youth and family members or other collateral strategies to minimize the negative effects of the symptoms which interfere with the child/youth's daily living and/or overall well-being.
  • Provide strength-based service planning - identifying strengths and needs, resources, natural supports, and developing goals and objectives to utilize personal strengths, resources, and natural supports to address functional deficits associated with their mental illness.
  • CPST services also may include the delivery of Rehabilitative Psychoeducation, strength-based service planning, and rehabilitative supports; these services may be provided by a Bachelor's or Master's level CPST provider.
  • Provide services to develop and implement social, interpersonal, self-care, and independent living skills.
  • Engage the child/youth and family/caregiver in ways that support the everyday application of treatment methods as described in the child's/youth's treatment plan.
  • Involve the family /caregiver in having an integral role in the support and treatment of the child/youth's behavioral health need when applicable.
  • Deliver services within a variety of permissible settings including but not limited to community locations where the child/youth lives, works, attends school, engages in services, and/ or socializers.
  • Understands different views, experiences, orientations, and cultural differences and takes into account when planning for treatment.
  • Complete progress notes are completed within a timely manner in accordance with regulatory requirements.
  • Participate in staff meetings. Perform on-call consultation duties as assigned.
  • Engage in department planning and goal attainment.
  • Provide strength-based service planning.

Education and Experience Requirements:

  • Bachelor's Degree or higher required to provide 3 of the 6 CPST services of: Rehabilitative Psychoeducation, strength-based service planning and rehabilitative supports -these services may be provided by a Bachelor's or Master's level CPST provider.
  • Master's Degree from an accredited program, Social Work, Psychology, Rehabilitation Counseling or related area required to provide 3 of the 6 services: Intensive Interventions(counseling), crisis avoidance (counseling), Intermediate Term crisis management (counseling).
  • Preferred Experience in providing therapy/treatment services for at-risk youth and families.
  • Valid and current NYS driver's license with a clean MVR.

Compensation: $60,000 annually
