We are currently seeking a Speech Language Pathologist (SLP).

Duties and Responsibilities:

  • Assess patients referred for speech therapy and rehabilitation services.

  • Provide direct patient care by evaluating speech, language, cognitive, and swallowing disorders.

  • Develop and implement individualized treatment plans based on physician orders and patient needs.

  • Establish realistic, measurable goals and monitor patient progress.

  • Complete evaluations, weekly progress notes, recertifications, and discharge documentation.

  • Educate patients, families, and caregivers on therapy techniques and treatment plans.

  • Collaborate with interdisciplinary team members to provide high-quality, patient-centered care.

  • Perform additional duties as assigned by the Director of Rehabilitation.

  • Maintain compliance with HIPAA, confidentiality requirements, and all applicable federal and state regulations.

Qualifications:

  • Graduate of an accredited Speech-Language Pathology program.

  • Current, valid state license to practice as a Speech-Language Pathologist.

  • Certificate of Clinical Competence (CCC-SLP) preferred, if applicable.

  • Long-term care and/or geriatric experience is preferred but not required.

  • Strong communication, documentation, and teamwork skills.
