We are currently seeking a Speech Language Pathologist (SLP).

Duties and Responsibilities:
Assess patients referred for speech therapy and rehabilitation services.
Provide direct patient care by evaluating speech, language, cognitive, and swallowing disorders.
Develop and implement individualized treatment plans based on physician orders and patient needs.
Establish realistic, measurable goals and monitor patient progress.
Complete evaluations, weekly progress notes, recertifications, and discharge documentation.
Educate patients, families, and caregivers on therapy techniques and treatment plans.
Collaborate with interdisciplinary team members to provide high-quality, patient-centered care.
Perform additional duties as assigned by the Director of Rehabilitation.
Maintain compliance with HIPAA, confidentiality requirements, and all applicable federal and state regulations.
Qualifications:
Graduate of an accredited Speech-Language Pathology program.
Current, valid state license to practice as a Speech-Language Pathologist.
Certificate of Clinical Competence (CCC-SLP) preferred, if applicable.
Long-term care and/or geriatric experience is preferred but not required.
Strong communication, documentation, and teamwork skills.
