Responsibilities:

• Provide direct patient care, including administering medications, wound care, and vital sign checks

• Collaborate with the nursing team to develop and implement individualized care plans

• Monitor and report any changes in resident condition to the Charge Nurse and/or physicians

• Assist with admissions, discharges, and transfers, ensuring a smooth transition for residents

• Maintain accurate and up-to-date records of resident care and services provided

• Participate in quality improvement initiatives to ensure the highest level of care for our residents

Requirements:

• Current LPN license in the state of Rhode Island

• High school diploma or equivalent required, nursing diploma or degree preferred

• Basic Life Support (BLS) certification required

• Ability to lift 50 pounds and perform duties that require frequent walking and standing
