Job Overview
Dedicated Support Network is seeking a compassionate and organized Medical Coordinator to oversee the healthcare needs of individuals with intellectual disabilities and autism within our community group homes. The ideal candidate will ensure residents receive optimal medical care by coordinating appointments, managing medications, maintaining compliant health records, and serving as the primary liaison between residents, families, and healthcare providers.
Duties
- Healthcare Coordination: Schedule, track, and transport residents to medical, dental, psychiatric, and specialist appointments using company vehicles.
- Medication Management: Oversee the ordering, receiving, and auditing of all resident medications while meticulously maintaining Medication Administration Records (MARs).
- Compliance & Documentation: Maintain accurate, up-to-date medical charts and health histories in strict compliance with Pennsylvania Chapter 6400 and Chapter 6500 regulations.
- Staff Training: Educate and train direct care staff on individual medical protocols, adaptive equipment, and proper medication administration.
- Liaison Duties: Serve as the central point of contact for physicians, pharmacies, families, and case managers regarding resident health updates.
- Emergency Response: Provide guidance during medical crises, coordinate urgent care, and document incidents according to protocol.
Qualifications
Education: High school diploma or equivalent required; an Associate’s or Bachelor’s degree in nursing, healthcare administration, or human services is preferred.
Experience: Minimum of 2 years of experience working with individuals with intellectual disabilities, autism, or in a residential healthcare setting.
Regulatory Knowledge: Familiarity with Pennsylvania Chapter 6400 and Chapter 6500 licensing regulations is highly preferred.
Certifications: Current CPR/First Aid certification is required;
Skills: Strong organizational skills, excellent verbal and written communication, and proficiency with electronic health records (EHR).
Requirements: Must pass a criminal background check, child abuse clearance, drug screening, and possess a valid driver's license with a clean driving record (driving is a mandatory requirement for this role).
