Job Overview
\nWe are seeking an experienced Registered Nurse (RN) Case Manager to join a leading healthcare organization in New Jersey. This field-based position focuses on coordinating care for members through comprehensive assessments, care planning, and collaboration with providers to improve health outcomes.
\nThe Case Manager will conduct both telephonic and face-to-face assessments in members' homes, hospitals, behavioral health facilities, and skilled nursing facilities while ensuring timely, cost-effective, and member-centered care.

Responsibilities
\n- \n
- Conduct comprehensive clinical assessments of members' healthcare needs. \n
- Develop, implement, and monitor individualized care plans. \n
- Coordinate care with physicians, hospitals, community providers, and caregivers. \n
- Perform home visits and facility visits as needed. \n
- Facilitate safe hospital discharges and transitions of care. \n
- Identify and address social determinants of health, including housing, transportation, and food insecurity. \n
- Maintain accurate and timely documentation in the electronic medical record. \n
- Apply case management guidelines, regulatory requirements, and clinical best practices. \n
- Collaborate with interdisciplinary healthcare teams to improve patient outcomes. \n
- Serve as a resource and mentor to new team members when appropriate. \n
Minimum Qualifications
\n- \n
- Active, unrestricted New Jersey RN License \n
- BSN required (or Licensed Social Worker (LSW) if applicable) \n
- Minimum 3 years of recent clinical experience in one of the following:\n
- \n
- Acute Care Hospital \n
- Home Health \n
- Ambulatory Care \n
- Skilled Nursing Facility \n
\n - Strong assessment and care coordination skills \n
- Excellent communication and documentation abilities \n
- Comfortable working independently in a field-based environment \n
- Proficient with electronic medical records and Microsoft Office \n
