Pride health is actively hiring a Registered Nurse to join our client's home health program in a growing care team serving patients.

Compensation:

- Local Pay: $50–$59/hour (W-2) \n
- Travel Pay: $2,500–$2,600 gross per week \n
- Mileage Reimbursement: $0.49/mile (from first patient to last patient) \n

Productivity expectations: 30 points
\nOn Call/Weekend Requirement: Yes, one weekend every 4 weeks with a day off the Tuesday following your weekend.
Coverage Areas: Thomas, Brooks, and Colquitt county
Shift schedule (days, start & end times): Monday-Friday 8:00pm-4:30pm, and one weekend every 4 weeks
Responsibilities:
\n- \n
- Deliver skilled home health nursing services, including patient education, disease management, wound care, and infection prevention; ensure timely, accurate documentation in Homecare Homebase (HCHB), maintain productivity standards (30 points/week), and coordinate care across Thomas, Brooks, and Colquitt counties. \n
- Supervise and provide clinical oversight to home health aides and LPN/LVNs, complete required supervisory visits, and ensure compliance with state, federal, payer, and agency policies to promote quality patient outcomes. \n
- Collaborate effectively with physicians, patients, families, and agency staff to coordinate care, communicate clinical updates, support quality improvement initiatives, reduce avoidable hospitalizations, and participate in on call/weekend rotations as required. \n
- Conduct comprehensive in-home nursing assessments, including OASIS evaluations and medication reconciliation, while developing, implementing, and updating individualized plans of care in collaboration with physicians and the interdisciplinary care team. \n
