Pride health is actively hiring a Registered Nurse to join our client's home health program in a growing care team serving patients.

Compensation:

\n

\n
• Local Pay: $53–$58/hour (W-2)
\n
• Travel Pay: $2,400–$2,550 gross per week
\n
• Mileage Reimbursement: $0.50/mile (from first patient to last patient)
\n

\n

Location: Abbeville, LA, 70510; Coverage Areas – VERMILLION PARISH.

\n

Schedule details (hours per day/week): MONDAY-FRIDAY WITH 1 WEEKEND PER MONTH, 08:00 AM to 04:30 PM

\n

Productivity expectations: 30 POINTS PER WEEK

\n

 

\n

On Call/Weekend Requirement: 1 WEEKEND PER MONTH, 1-2 DAYS PER WEEK

\n

Responsibilities:

\n

\n
• Conduct comprehensive Home Health OASIS assessments, admissions, recertifications, and routine skilled nursing visits while developing and updating individualized patient care plans in accordance with physician orders and agency guidelines.
\n
• Provide high-quality in-home nursing care across Vermilion Parish, accurately document all patient encounters using PointCare/Homecare Homebase (HCHB), coordinate with the interdisciplinary care team, and participate in the on-call/weekend rotation as scheduled.
\n

Must have qualifications/experience: HOME HEALTH EXPERIENCE, POINTCARE/HCHB EXPERIENCE.

Type of certification required: RN LICENSE TO PRACTICE IN LOUISIANA 1 YEAR EXPERIENCE

Dress code/scrub color: NAVY BLUE
