• Patient Ratios: Case Manage 20-25 patient with an expectation of 6 visits per day; may use as re-vistor if agency needs change at any time during contract

\n • Equipment: clinician will be assigned an agency lap top and I phone with case and chargers, mouse, a nursing supply bin, nursing bag with vital signs equipment and an INR machine along with agency bag. All must be returned upon departure.
