DayOne Staffing, Inc. is seeking a travel nurse RN Case Manager, Hospice Palliative Care RN for a travel nursing job in Grand Junction, Colorado.

- Provide compassionate, patient-centered hospice nursing care to patients in their homes, assisted living facilities, nursing homes, and group homes. \n
- Perform comprehensive patient assessments and develop individualized plans of care based on patients' goals and clinical needs. \n
- Manage an assigned caseload of hospice patients and coordinate all aspects of their care. \n
- Lead and collaborate with the interdisciplinary care team to ensure high-quality, holistic patient care. \n
- Monitor, assess, and manage pain and other symptoms to maximize patient comfort and quality of life. \n
- Educate patients and family caregivers on disease progression, symptom management, medications, and end-of-life care. \n
- Support patients and families in identifying and achieving their personal goals of care. \n
- Administer medications, treatments, and interventions in accordance with physician orders and hospice protocols. \n
- Accurately document patient assessments, care plans, interventions, and clinical outcomes in the electronic medical record. \n
- Coordinate with physicians, social workers, chaplains, therapists, and other healthcare professionals to ensure continuity of care. \n
- Provide emotional support, guidance, and counseling to patients and their families throughout the hospice journey. \n
- Respond promptly to changes in patient condition and adjust care plans as needed. \n
- Participate in weekday coverage (Monday–Friday, 8:00 AM–5:00 PM) with occasional weekend backup and on-call responsibilities. \n
- Ensure compliance with hospice regulations, infection prevention standards, and organizational policies. \n
- Advocate for patient dignity, comfort, and quality of life while delivering \n
