Essex Home Health is hiring a Full‑Time LPN to provide home‑health visits for approximately 40 patients per week, focusing on hypertension and diabetic care.

Key Duties

• Complete home‑health nursing visits (40 patients weekly)
  
  
• Provide hypertension and diabetic patient care
  
  
• Perform assessments, vitals, medication teaching
  
  
• Document visits accurately and timely
  
  
• Communicate patient changes to RN Case Manager
  
  
• Work every other weekend
  
  
• Travel between Lawrence, Haverhill, and Methuen (mileage reimbursement)
  
  

Requirements

• Active LPN license in Massachusetts
  
  
• Home‑health experience preferred
  
  
• Strong communication and patient‑education skills
  
  
• Reliable, organized, and able to work independently
