Assistant Director of Nursing/ MDS Coordinator

The Pavilion at Stow is currently seeking a dedicated and skilled RN to take on the dual role of Assistant Director of Nursing and MDS Coordinator. This key leadership position blends clinical oversight with regulatory and reimbursement responsibilities in a supportive and collaborative environment.

Position Overview

You'll support the Director of Nursing in managing daily nursing operations while leading the MDS (Minimum Data Set) process to ensure accurate assessments and optimal reimbursement.

Key Responsibilities

• Support DON with nursing department operations, rounding, care standards, and staff development 

• Oversee completion and submission of all MDS assessments, care plan development, and quality measures per CMS guidelines

• Serve as liaison between nursing, therapy, and medical staff to ensure care coordination based on MDS findings

• Monitor regulatory compliance and prepare the facility for successful surveys 

• Conduct staff education on clinical documentation, regulatory changes, and best practices

• Analyze clinical data and drive quality improvement initiatives

What You’ll Bring

• Active RN license in Ohio

• Current experience as an ADON or MDS Coordinator in a skilled nursing facility or demonstrated readiness to learn both components

• Strong clinical judgment, leadership aptitude, and understanding of MDS 3.0/PDPM

• Excellent communication, data analysis, and training skills

• Minimum of 3 years of supervisory experience recommended
