Key Duties

• Complete home‑health nursing visits ($30 per visit)
  
  
• Focus on hypertension and diabetic patient care
  
  
• Perform assessments, vitals, medication teaching
  
  
• Provide disease‑management education
  
  
• Document visits accurately
  
  
• Communicate patient changes to RN Case Manager
  
  

Requirements

• Active LPN license in Massachusetts
  
  
• Home‑health experience preferred
  
  
• Strong patient‑education and communication skills
  
  
• Reliable, professional, and able to work independently
