Responsibilities

• Provide direct patient care to residents in a variety of settings, including skilled nursing facilities, rehabilitation programs, and post-acute care settings

• Administer medications, treatments, and procedures as ordered by physicians and as part of a collaborative care team

• Monitor patient conditions, report changes, and document patient information accurately and thoroughly

• Collaborate with physicians, nurses, and other healthcare professionals to develop and implement individualized patient care plans

• Educate patients, families, and caregivers on self-care techniques, disease prevention, and health promotion

• Participate in quality improvement initiatives and contribute to the development of policies and procedures

Requirements

• Current licensure as a Licensed Practical Nurse (LPN) in the state of Pennsylvania

• High school diploma or equivalent required; associate's degree or diploma preferred

• Ability to work in a fast-paced environment and prioritize tasks effectively

• Strong communication and interpersonal skills, with the ability to work collaboratively with colleagues and families

• Familiarity with electronic medical records and basic computer skills
