Licensed Practical Nurse (LPN)

**Job Summary:**
The Licensed Practical Nurse (LPN) provides compassionate, patient-centered nursing care under the supervision of a Registered Nurse or physician. The LPN administers medications and treatments, monitors patient conditions, performs routine nursing procedures, documents care, and collaborates with the interdisciplinary healthcare team to promote safe, effective, and high-quality patient outcomes.

**Requirements:**

* Graduate of an accredited Practical Nursing program.
* Current Licensed Practical Nurse (LPN) license in the state of practice.
* Current BLS certification preferred or required per facility policy.
* Recent experience in long-term care (LTC), skilled nursing (SNF), rehabilitation, clinic, or acute care preferred.

**Responsibilities:**

* Provide direct nursing care according to individualized care plans.
* Administer medications, treatments, and therapeutic interventions as prescribed.
* Monitor, assess, and report changes in patient condition to the Registered Nurse or provider.
* Assist with admissions, transfers, discharges, and daily patient care activities.
* Maintain accurate, timely, and complete clinical documentation.
* Collaborate with physicians, Registered Nurses, and the interdisciplinary healthcare team.
* Educate patients and families within the scope of practice.
* Follow infection prevention, patient safety, HIPAA, and regulatory standards.
* Perform other duties as assigned.
