Responsibilities
- Provide and document nursing care for patients in their homes, including assessment, planning, intervention, and evaluation of care.
- Intervene to promote, maintain, or restore health and support rehabilitation in the home setting; educate patients and families on disease management and self-care.
- Collaborate with physicians, other clinicians, and community resources to coordinate comprehensive home-based care.
- Apply nursing protocols and maintain documentation; utilize independent judgment within approved guidelines; protect patient confidentiality.
- Manage time, communicate effectively with the care team, and adapt care plans as patient needs evolve; participate in on-call rotation as required.

Requirements
- Licensure: Massachusetts LPN license (active) required; Massachusetts Temporary Nursing Service Agency License (Nursing Pool) required to staff for temporary needs and must be uploaded with the traveler profile.
- Experience: Minimum 1 year of acute nursing care; hospital-based experience preferred; home health experience required; infusion therapy experience preferred; Certified Home Health with proficiency in OASIS highly preferred.
- Certifications: American Heart Association BLS or American Red Cross CPR required.
- Education: LPN credential and MA licensure; relevant continuing education as applicable.
- Driver’s license: Valid MA driver’s license with insurance required; traveler must have own vehicle.
- Location/bilateral constraints: CCTHIS client will not consider candidates residing within 50 miles of the facility; profile must reflect a permanent address and commute considerations.
- Profile/document requirements: MA LPN license uploaded; Massachusetts Temporary Nursing Service Agency License uploaded; Skills Checklist for Home Health (LPN) current within 1 year; Supervisory reference from within the past two years; cover page and any required documents as specified; RTO details must be noted in profile with no more than 5 days of time off requested.
- Scheduling/flexibility: Willingness to be flexible regarding work hours; on-call responsibilities may be required.
