Tobacco Root Mountains Care Center is seeking an experienced Registered Nurse (RN) to serve as a Director of Nursing (DON) / MDS Nurse in its skilled nursing facility. The facility is prioritizing candidates with Director of Nursing experience combined with MDS experience. The RN will oversee nursing operations, ensure regulatory compliance, coordinate resident care, manage MDS assessments, and lead the nursing team to deliver high-quality long-term care.

• Director of Nursing (DON) experience strongly preferred

• MDS (Minimum Data Set) experience required/preferred

• Long-Term Care (LTC) or Skilled Nursing Facility experience required

• Leadership and staff management experience

• Strong knowledge of CMS regulations and long-term care compliance

• Resume must clearly demonstrate all relevant nursing and leadership experience

• Active Registered Nurse (RN) License

• Current BLS Certification

• Long-Term Care experience

• MDS Coordinator experience preferred

• Director of Nursing leadership experience strongly preferred

• Tobacco Root Mountains Care Center (TRMCC)

• 38-bed Skilled Nursing Facility

• Current census: 13 residents

• Housing provided

• Variable day shifts

• 40 hours per week
