Position Overview
We are seeking a dedicated Registered Nurse (RN) to join our team full-time as a Field Nurse supporting patients across Manhattan and the Bronx. Key Responsibilities
- Perform in-home skilled nursing assessments and visits
- Develop, manage, and update individualized care plans
- Educate patients and caregivers on conditions, medications, and self-care practices
- Document care accurately and in compliance with regulations
- Collaborate with interdisciplinary healthcare teams to ensure quality outcomes
- Supervise home health aides when necessary
Schedule
- Monday through Friday
- 8:00 AM – 5:00 PM
- Field-based travel across Manhattan and the Bronx
Qualifications
- Active New York State Registered Nurse (RN) license
- 1–2 years of nursing experience in home care, long-term care, or a related setting preferred
- Excellent clinical judgment and communication skills
- Reliable transportation for travel within boroughs
- Bilingual in Spanish or Mandarin is a plus
- Professional liability insurance is required
