Registered Nurse PRN is responsible for traveling to a patient’s home to ensure care is administered effectively by their Personal Care Worker (PCW) while the patient maintains their independence. Additionally, provides clinical knowledge and use of interpersonal skills to answer questions from clients, families, and employees to resolve issues involving nursing care.

Responsibilities:


· Identifies patient service requirements by establishing personal rapport with potential and actual patients to understand service requirements.

· Assess and charts observations of the patients’ home health care at each visit.

· Communicate and coordinate with Physicians, Physical Therapists, Occupational Therapists, and other individuals in the patients’ home health care plan.

· Complete evaluation tasks to adhere to all Wisconsin home health care laws and regulations.

· Maintains proper recordkeeping and maintenance of patient records adhering to governmental requirements.

· Coordinates admissions, assessments, and discharges of clients with required staff.

· Provides education to patients and families on proper home health care procedures and strategies.

Competency Experience:


Current RN license in the State of Wisconsin.

2-3 years of experience preferably in home care services.

Exhibits advanced professional knowledge and leadership skills.

Excellent communication and interpersonal skills.

Strong organizational and time management skills, and ability to prioritize.

Travel required, must have a valid Wisconsin Driver’s License.

Additional pay incentives include In-home Nurse Assessment Fees set at $130 per annual assessment, $50 per Supervisory Visit assessment plus mileage reimbursement.
