Service Areas:
- Dallas
- Duncanville
- Mansfield
- McKinney
- Allen
- Conduct comprehensive patient assessments and develop individualized care plans
- Provide skilled nursing care and symptom management
- Coordinate care with physicians, social workers, chaplains, and other team members
- Educate patients and families on disease progression, medications, and care techniques
- Monitor patient condition and adjust care plans as needed
- Maintain accurate and timely clinical documentation
- Participate in on-call rotation as needed (PRN basis)
- Ensure compliance with hospice regulations and standards
- Current and unrestricted RN license in the state of Texas
- Minimum 1–2 years of hospice, home health, or related experience preferred
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal abilities
- Ability to work independently and manage time effectively
- Reliable transportation and valid driver’s license
- Current CPR certification (required)
