Shift: Days-No Nights, Weekends or Holidays!
The Nurse Navigator serves as the primary guide for orthopedic surgery patients throughout their care journey. This role coordinates patient care, provides education and support, helps with discharge planning, and works closely with physicians, nurses, and other healthcare team members to ensure excellent patient outcomes.
Responsibilities
- Coordinate and manage patient care from pre-op through recovery and follow-up.
- Provide patient and family education.
- Conduct inpatient rounding and assist with discharge planning.
- Collaborate with physicians, nurses, and support services.
- Monitor quality, patient outcomes, and length-of-stay metrics.
- Maintain accurate documentation in EPIC and utilization management systems.
- Serve as a clinical resource and mentor to staff.
- Support program growth, physician relationships, and community outreach.
Qualifications
Required:
- Graduate of an accredited nursing program
- Current RN license
- BLS certification
- 2–3 years of nursing experience
Preferred:
- BSN
- ACLS certification
- Orthopedic, care coordination, navigator, or case management experience
Physical Requirements
- Lift, push, or pull up to 50 lbs.
- Prolonged standing and walking
- Frequent bending, kneeling, reaching, twisting, and squatting
