Position Summary
The Director of Nursing (DON) is responsible for the overall clinical oversight, supervision, and coordination of patient care services within the home health agency. This role ensures compliance with Medicare Conditions of Participation, state and federal regulations, agency policies, and professional standards of nursing practice.
The DON provides leadership to clinical staff, supports quality improvement initiatives, and ensures safe, effective, and compliant delivery of home health services.
Reporting Structure
This position reports to the Administrator and works collaboratively with agency leadership, including quality assurance, therapy, intake, billing, and operational teams. The DON provides direct oversight of nursing staff and clinical operations across the organization.
Scope of Practice
The DON functions within the scope of Florida RN licensure and serves as the agency’s designated clinical leader responsible for nursing services oversight. This role includes supervision of clinical staff, oversight of patient care delivery, policy implementation, and regulatory compliance across clinical operations.
Essential Job Functions
1. Clinical Leadership & Oversight
- Provide leadership and oversight of all clinical services and nursing operations within the agency
- Ensure patient care is delivered safely, effectively, and in accordance with physician orders and care plans
- Oversee nursing practice standards and ensure compliance with applicable regulations and agency policies
- Support appropriate clinical staffing and supervision across patient care services
2. Compliance & Regulatory Readiness
- Ensure ongoing compliance with Medicare Conditions of Participation and applicable state and federal regulations
- Maintain readiness for ACHC surveys, audits, and regulatory inspections
- Oversee implementation of clinical policies, procedures, and corrective action plans
- Support infection control, patient safety, and quality assurance standards across clinical operations
3. Quality Improvement & Performance Monitoring
- Monitor clinical quality indicators, OASIS outcomes, and patient care performance metrics
- Support quality improvement initiatives and corrective action planning
- Review clinical documentation and operational trends to identify improvement opportunities
- Promote continuous improvement across clinical departments
4. Staff Supervision & Development
- Supervise Registered Nurses, Licensed Practical Nurses, Certified Nursing Assistants, and other clinical personnel as applicable
- Support onboarding, training, competency validation, and performance management of clinical staff
- Provide clinical guidance and leadership to field staff and interdisciplinary teams
- Participate in recruitment and retention efforts for nursing and clinical personnel
5. Interdisciplinary Collaboration
- Work closely with the Administrator and department leaders to support coordinated agency operations
- Collaborate with physicians, therapists, intake, billing, and quality teams to ensure continuity of care
- Participate in care conferences, leadership meetings, and strategic planning initiatives
Qualifications
- Bachelor’s degree in Nursing (BSN required; MSN preferred) required. Current Registered Nurse (RN) license in good standing required. Minimum of five (5) years of clinical nursing experience required, with at least two (2) years in home health and two (2) years in a supervisory or nursing leadership role required.
- Strong knowledge of Medicare Conditions of Participation and home health regulations required
- Experience with OASIS, clinical operations, and survey readiness strongly preferred
- Current CPR certification required
- Valid driver’s license, reliable transportation, and auto insurance as applicable
- Ability to pass background screening and all compliance requirements

Physical Requirements
- Ability to sit, stand, and work in office and clinical environments as needed

- Effective communication with staff, patients, families, physicians, and leadership
