Remote Patient Monitoring (RPM) Clinical Coordinator

Wichita Nephrology Group is seeking a motivated, patient-focused, and tech-savvy Remote Patient Monitoring (RPM) Clinical Coordinator to join our growing care management team.

The RPM Clinical Coordinator supports patients enrolled in our Remote Patient Monitoring program by onboarding patients, setting up monitoring devices, assisting with smartphone applications, reviewing daily patient readings, and communicating significant trends to licensed providers. This position plays a vital role in helping patients successfully participate in the RPM program while supporting providers with timely and accurate information.

This role does not diagnose medical conditions, provide medical advice, interpret clinical findings, or make treatment recommendations.

Essential Responsibilities

• Enroll patients into the Remote Patient Monitoring program.
• Set up RPM devices and pair them with the patient's smartphone or monitoring hub.
• Link devices to the electronic medical record (eClinicalWorks) and verify successful transmission of patient readings.
• Educate patients on proper use of monitoring equipment and mobile applications.
• Assist patients with troubleshooting Bluetooth connectivity, mobile apps, syncing issues, passwords, and device-related concerns.
• Review daily patient monitoring reports for missing readings, abnormal values, and trends using established protocols.
• Notify physicians or licensed clinical staff of significant trends, abnormal readings, or other findings requiring clinical review.
• Document all patient interactions, technical support, and communications accurately in the electronic medical record.
• Escalate technical or clinical concerns through established workflows.
• Encourage patient engagement and compliance with the RPM program.
• Work collaboratively with providers and staff to improve patient outcomes and ensure a positive patient experience.
• Maintain HIPAA compliance and protect patient confidentiality.

Qualifications

• Previous experience in healthcare, care coordination, patient services, or Remote Patient Monitoring preferred.
• Excellent telephone etiquette and interpersonal communication skills.
• Strong technical aptitude and confidence using smartphones, Bluetooth devices, mobile applications, and connected medical devices.
• Ability to explain technology in simple, patient-friendly language.
• Strong attention to detail and organizational skills.
• Comfortable reviewing patient monitoring reports and identifying trends that require provider notification.
• Experience with eClinicalWorks or another electronic medical record system preferred.
• Ability to work independently while managing multiple priorities.

Scope of Position

The RPM Clinical Coordinator is responsible for supporting patient participation, device connectivity, and monitoring workflows. This position follows established clinical protocols and communicates patient trends to licensed providers but does not provide medical advice, diagnose conditions, interpret clinical findings, or recommend treatments or medication changes.
