Job Description

New England Healthcare Solutions LLC is seeking a motivated Registered Nurse (RN), BSN preferred, to support our Adult Foster Care (AFC) and Group Adult Foster Care (GAFC) programs.

This is a field-based/home health role focused on opening AFC and GAFC cases, completing assessments, assisting with MDS and prior authorizations, and helping members remain safely at home.

We are especially interested in candidates with a BSN and leadership experience who may also qualify to assist with AFC Program Director responsibilities.

Responsibilities

  • Open and manage AFC and GAFC cases
  • Complete initial nursing assessments and reassessments
  • Assist with MDS assessments
  • Support prior authorizations for MassHealth, CCA, and other payers
  • Develop and update individualized care plans
  • Conduct home visits and clinical evaluations
  • Collaborate with caregivers, care managers, providers, and office staff
  • Maintain accurate and timely clinical documentation
  • Support compliance with AFC/GAFC program requirements
  • Assist with program operations, staff support, and quality assurance as needed

Requirements

  • Active Registered Nurse license in Massachusetts
  • BSN strongly preferred
  • At least 2 years of recent experience caring for elders or adults with disabilities preferred
  • Strong assessment and documentation skills
  • Ability to work independently in the community
  • Reliable transportation
  • Professional communication skills

Preferred Qualifications

  • BSN with healthcare leadership, supervisory, or administrative experience
  • AFC or GAFC experience
  • Home health or community nursing experience
  • MDS experience
  • Prior authorization experience
  • MassHealth, Medicaid, or CCA experience
  • Bilingual English/Spanish preferred

Program Director Preferred Qualification

Candidates who meet AFC Program Director qualifications are strongly preferred. This includes a bachelor’s degree, at least 5 years of recent professional healthcare experience working with elders or adults, and at least 1 year in an administrative, supervisory, or leadership role.
