Job Title: Nurse Practitioner – Nursing Home

Location: Orange, NJ

Job Type: Full-Time

Salary Range: $135,000 - $150,000

Job Summary:

We are seeking a compassionate Nurse Practitioner to provide primary care services to residents in skilled nursing and long-term care facilities. This is an excellent opportunity for both experienced providers and new graduates who are passionate about delivering high-quality, patient-centered care in a collaborative environment.

Key Responsibilities:

• Perform comprehensive physical examinations and routine follow-up visits.
• Diagnose and treat acute and chronic medical conditions.
• Develop, implement, and monitor individualized treatment plans.
• Order and interpret laboratory tests, diagnostic imaging, and other studies.
• Prescribe medications and treatments in accordance with state regulations.
• Manage chronic diseases and provide preventive care.
• Maintain accurate and timely documentation in the EMR/EHR.
• Collaborate with physicians, nursing staff, and interdisciplinary healthcare teams.
• Educate patients, families, and caregivers on disease prevention and treatment plans.
• Refer patients to specialists when appropriate.

Qualifications:

• Master's Degree in a Nurse Practitioner Program: Family or Adult Geriatrics
• Current New Jersey State License as a Nurse Practitioner.
• National Board Certification require.
• American Association of Nurse Practitioners (AANP)
• American Nurses Credentialing Center (ANCC)
• Oncology Nursing Certification Corporation (ONCC)
• Bilingual (Spanish Speaking) is a plus.
• EMR/EHR experience a plus.
