American Traveler is seeking an experienced RN for a Home Health Visit Nurse position in the Port Angeles and Sequim areas, requiring a valid WA RN license and home health or acute care experience.

- Home health care setting serving patients in their place of residence in the Port Angeles and Sequim areas \n
- Post-acute care patient population with a variety of nursing needs as delegated by a Case Manager RN \n
- Full-time schedule of 5x8-hour day shifts, 40 hours per week \n
- Weekend and holiday coverage required \n
- 13-week contract \n
- Charting performed in Epic (experience required) \n

- Active WA State RN license or compact license accepted in WA currently in hand \n
- Current BLS certification \n
- Associate's degree required; BSN preferred \n
- Minimum of 1 year of recent experience in home health, acute, or long-term care \n
- Valid WA driver's license with a driving record in compliance with facility standards \n
- Proof of automobile insurance and reliable personal vehicle for traveling to patient residences \n
- Cell phone with activated voicemail required \n
- Home internet connectivity required (work laptop provided) \n

- Provides nursing visits and continuity of care for a caseload of patients under the overall planning and coordination of the Case Manager RN \n
- Delivers nursing care in patients' places of residence in accordance with physician orders and applicable regulations \n
- Keeps the Case Manager RN informed of patient progress, recommended actions, physician orders, and plans for upcoming visits \n
- Expected to apply full working knowledge of nursing principles and demonstrate independent problem-solving in complex and diverse situations \n
- Works as part of an interdisciplinary care team \n
