Description

\n As an Outpatient RN Care Manager, you will manage chronic diseases and monitor medical conditions in collaboration with the interdisciplinary team and primary care providers. You will provide caregiver education and support within a licensed adult day health program, focusing on reducing hospitalizations and improving patient outcomes.

Requirements\n

• EMR System experience preferred.
\n
