Description\n

American Traveler is seeking an experienced Home Health RN for a 13-week travel contract in California, requiring a valid CA RN license, BLS certification, and a valid driver's license with insurance.

  • Home health care setting, providing in-home nursing visits to recently discharged patients at high risk for rehospitalization
  • \n
  • Primary patient population includes adults with CHF, COPD, wounds (including wound vacs), and IV/PICC line management; no pediatric patients
  • \n
  • Average caseload of approximately 5 visits per day, based on a point system (start of care: 2.5 pts, recertification: 1.5 pts, resumption of care: 1.75 pts, routine visit: 1 pt), totaling 5.25 points per day
  • \n
  • Charting performed in Cerner EMR
  • \n
  • 8-hour day shifts, 5 days per week; schedule runs either Monday–Friday or Tuesday–Saturday (Tuesday–Saturday preferred)
  • \n
  • Every other weekend (EOW) required
  • \n
  • Holiday coverage needed, particularly for Thanksgiving and/or Christmas
  • \n
  • No on-call or standby requirements
  • \n
  • Floating within home health only
  • \n
Requirements\n

  • Active CA RN license, fully active at time of consideration — pending licenses not accepted
  • \n
  • Active BLS certification required — pending certifications not accepted
  • \n
  • Valid driver's license and personal auto insurance required
  • \n
  • Minimum 2 years of RN experience required
  • \n
  • Experience with HomeCare HomeBase (HCHB) preferred
  • \n
  • 2 professional references required: one supervisor from within the last 12 months worked, and one peer or supervisor from within the last 3 years worked, including dates of employment and eligibility for rehire
  • \n
  • Driver's license required for consideration
  • \n

  • Conduct in-home nursing assessments, start-of-care visits, recertifications, resumption-of-care visits, and routine visits
  • \n
  • Patient education related to respiratory equipment (CPAP, BiPap, vents) is performed as part of standard care
  • \n
  • A home health aide is part of the department team
  • \n
  • Supervisor (Holly Haynes) is available during working hours for routine questions and after hours for urgent needs; an administrator on call is also available after hours and on weekends
  • \n
  • Schedule is typically set by the previous day, but morning changes are common due to unprocessed start-of-care orders or sick calls
  • \n
  • Any work-appropriate scrubs are acceptable
  • \n
  • Candidates must not have been directly employed (full-time, part-time, or PRN) by any CommonSpirit, CHI, or Dignity-affiliated facility within the past year; candidates employed PRN through outside staffing firms will be considered
  • \n
  • RNs at CommonSpirit Sacramento-region facilities must pass the Relias Dysrhythmia exam (basic or advanced) prior to an offer being confirmed
  • \n
  • A pass/fail sterile technique test for Foley catheter insertion and central line dressing changes is administered on the first day of orientation
