Pay: $105,000.00 - $110,000.00 per year
Directly Supervises and Evaluates: Coordinates care provided by other professionals, and manages services provided by paraprofessionals serving patients
Description: Responsible for organizing and directing home care services to patients. Ensures appropriate coordination among all professional and paraprofessional services related to the care provided to the patient. Furnishes those services requiring substantial and specialized nursing skill. Must have NYS RN License.
JOB DUTIES /KNOWLEDGE
Completes an initial assessment of the patient and family to determine home care needs
Completes a comprehensive physical and psychosocial assessment of the patient
Performs a comprehensive medication review, initiates and updates the Medication Profile as required
Assesses home safety and makes suggestions when hazards are noted
Arranges for services / supplies ordered and manages the coordination of other professional and paraprofessional services in the home
Oversees the completion of the comprehensive Plan of Care in collaboration with the patient’s physician and other disciplines.
Ensures cost effective utilization of service and quality of care by revising the treatment plan as necessary
Performs all required OASIS documentation accurately and completely within the prescribed time frames.
Performs a comprehensive patient reassessment as required but at a minimum of every sixty days.
Provides the prescribed or emergency treatment in the home, ensuring physician orders are current
Initiates appropriate preventive and rehabilitative nursing services.
Develops teaching strategies for patient/family/caregiver, providing instructions using accepted principles of learning
Develops the Paraprofessional Plan of Care and effectively supervises paraprofessional staff servicing the patient as per agency policy
Demonstrates sound judgment and independent problem solving skills
Maintains knowledge of third party reimbursement criteria in planning the care
Communicates effectively to other professionals within the agency and the patient’s physician and vendors
Treats patients with dignity and respect and maintains a non-discrimination policy
Maintains current, complete, timely and appropriate documentation; updates medical orders
Obtains and utilizes available resources to provide the highest quality patient care
Plans activities necessary for effecting discharge/transfer of patients
Provides instruction and intervention to implement effective infection surveillance and control.
Submits clinical notes within time frames required by Agency
Develops an appropriate discharge plan. Identifies impediments and adjusts accordingly. Notifies all parties with agency guidelines
Participation in the agency’s education, and quality and performance improvement activities
Assists in the orientation and education of other nursing personnel as requested
Collaborates with other clinical and business staff, other agency departments and outside agencies to achieve agency goals
Takes responsibility for maintaining professional compliance, related medical and other related forms
Participates in on-call duties as defined by Agency policy.
QUALIFICATIONS
Graduate of an accredited school of nursing. Current Registered Nurse licensure and registration in NY State
One year of clinical experience preferred
Home care experience preferred
Experience with electronic medical record preferred
Excellent verbal and written communication skills
Excellent clinical skills
