Occupational Therapist

Job Summary: 

Our agency is expanding, and we are looking for an experienced Occupational Therapist to provide home health services focused on improving patient independence and safety within their home environment. Responsibilities: 

• Conduct in-home evaluations and functional assessments 

• Develop and implement individualized treatment plans 

• Assist patients with ADLs (bathing, dressing, grooming, toileting) 

• Train patients on adaptive equipment and home safety modifications 

• Complete OASIS documentation as required 

• Collaborate with nurses, PTs, and other team members 

Qualifications: 

• Graduate of an accredited Occupational Therapy program 

• Active Florida OT license 

• Home health experience preferred 

• OASIS experience strongly preferred 

• Strong documentation and time management skills 

• Valid driver’s license and reliable transportation
